Dear Parents! The team of the Stem Cell Bank provides you with assistance and support from the very moment of deciding on collecting umbilical cord blood for your child!
 | 1. If you are interested in collecting and storing umbilical cord blood, we encourage you to contact us. All your questions will be answered. |  | 2. If you are not able to meet us in person, we will send you all the necessary information, including the agreement form. |  | 3. Send two copies of signed agreement form enclosing a registration fee payment slip. We will immediately send you a collection kit, which you should keep safe until delivery. |  | 4. You should inform the hospital where you are going to deliver your baby about your plan to collect umbilical cord blood. |  | 5. When you go into labour, bring the collection kit with you to the hospital and give it to your attending midwife. |
 | 6. After the baby is delivered and the umbilical cord is detached, your attending physician or nurse will collect the cord blood. |  | 7. The hospital will inform us about the cord blood collection. Hospital should inform us about the umbilical cord blood collection. We expect such an information also from you to be sure that the blood will be transported to the Bank as soon as possible. |  | 8. The cord blood of your baby will be transported to the Children's Memorial Health Institute in Warsaw. |  | 9. In our laboratory, the cord blood will be carefully tested, processed and frozen. |  | 10. Finally, you will receive a Certificate confirming storage of stem cells of your newborn baby in our Bank. The Certificate will include the basic information and test results of the collected blood. |
